Healthchek is a free voluntary preventative health care program for those on Medicaid for ages birth through 20 years of age providing the following services:
- Periodic examinations assessing both physical and mental health development
- Services necessary for the diagnosis and treatment of disease, illness or injury
- Vision services
- Dental services
- Hearing services
- Behavioral services

Healthchek screening results may be used for daycare, sports programs, Head Start, summer camp and school requirements. The screening physician may refer the child to a specialist for further diagnosis and/or treatment as needed.
Support services including:
- Provision of names of doctors, dentists and other providers
- Assistance with scheduling appointments
- Assistance with transportation to medical appointments
- Childhood lead poisoning prevention

How to Apply
Healthchek services are automatically provided with the approval of a Medicaid card. You may request information on additional information such as doctor names, by completing a Healthchek Assessment form.
